Selcted Haj Pilgrims need to deposit the following items at the Haj house

1. Original Passport

2. Two PP size photos with WHITE background only

3. Medical Certificate duly signed/seal by doctor in the format given
on the last pages of the Hajj Brochure. Do not forget to put the applicant signature on this form.

4. Original Advance Fee payment receipt.

5. Original CBC Test Report

6. Original X-Ray report of the radiologist/doctor (Not the XRay film but a report indicating the report of the XRay. This should be other than the medical certificate submitted)

7. Original Blood group test report.
